A police record or written certification that a foreign worker is not a criminal offender or has been examined for criminal liability issued by a foreign competent authority. In case a foreign worker has resided in Vietnam, only the police record issued by Vietnamese authority shall be required.<\/p>\n<p>The police record shall not exceed 06 months from the date of issuance to the date of submission of the dossier.<\/p>\n<p>2. A health certificate or medical examination certificate issued by a competent foreign or Vietnamese health authority or organization is valid for 12 months from the date of conclusion on the health to the date of submission of the dossier. In cases where foreigner is residing in Vietnam, one of the following hospitals in Ho Chi Minh City shall be granted:<\/p>\n<p>\u2013People&#8217;s Hospital 115;<\/p>\n<p>\u2013Van Hanh General Hospital.<\/p>\n<p>&#8211; Cho Ray Hospital.<\/p>\n<p>3. Documentation proving to be the manager, the chief executive officer, the expert or the technical worker.<\/p>\n<p>4. 02 (two) color photos (size of 4cm x 6cm, bare head, straight, visibility of face, two ears, no glasses, white background), photos taken no more than 06 (six) months<\/p>\n<p>5. A written request for the issuance of a work permit from the employer<\/p>\n<p>6. A certified copy of passport or valid paper in lieu of passport or valid international travel document as prescribed by law.<\/p>\n<p><strong>Duration<\/strong>: 30 days<\/p>\n<p><em><strong>Note<\/strong>: The police record, diplomas, certificates of experience, health certificate must be in consular legalization.
